Major skin/wounds conditions
Pressure Injury
Pressure injuries: localized skin/tissue damage from sustained pressure, friction, or shear. NPUAP staging I-IV + unstageable + deep tissue injury. Braden scale for risk. Repositioning q2h is standard.
Cellulitis
Cellulitis is non-purulent spreading bacterial infection of dermis and subcutaneous tissue. Most common: S. aureus and Group A Strep. Classic: erythema, warmth, swelling, tenderness. Mark borders to monitor spread.
Necrotizing Fasciitis
Necrotizing fasciitis (NF) is rapidly spreading infection of fascia and subcutaneous tissue — surgical emergency. LRINEC score. Severe pain out of proportion, crepitus, skin necrosis. Surgical debridement within 12-24 hours.
Burns
Burn severity: depth (superficial, partial, full thickness), TBSA (rule of nines), location. Parkland formula for fluid resuscitation in first 24h. Inhalation injury significantly increases mortality.
DFU
Diabetic foot ulcers affect 15-25% of people with diabetes. Wagner classification guides management. Neuropathy (most common), ischemia, and infection are the key factors. Leading cause of non-traumatic amputation.
