Start antibiotics within 1 HOUR: Piperacillin-Tazobactam 4.5g IV q6h (first-line) Add Vancomycin (suspected MRSA/line infection) Add antifungal (Micafungin) if prolonged neutropenia >5d G-CSF (Filgrastim/Pegfilgrastim) — prophylaxis Reverse isolation/neutropenic precautions
